As UK taxpayers face a huge bill as a result of errors made in the West Coast rail franchise, the reasons for the failure are emerging.
Several senior civil servants have been suspended over flaws in the franchising process which saw two main bidders offered conflicting information on how to assess risk and costs when bidding for the franchise. This has raised questions over conflict and accountability in the civil service and two independent reviews have been called.
